When life gets hard — when you're facing eviction, sleeping in your car, leaving a hospital with nowhere to go, or just trying to figure out what help exists — the last thing you need is a confusing website that doesn't answer your questions. So let's keep it simple.
What We Do
Mysti's Adult & Family Services is a community-based social services organization serving six California counties. We specialize in:
Housing Navigation — We help you find housing, apply for programs, work with landlords, and get move-in assistance.
Case Management — A dedicated case manager works with you to create a plan, connect you to services, and follow up until you're stable.
Enhanced Care Management (ECM) — Through CalAIM, we coordinate your health care, mental health, housing, and social needs.
Medi-Cal Enrollment — We help you apply for Medi-Cal and connect to covered services.
Community Supports — Practical assistance including housing deposits, medically supportive food, and benefit coordination.
Free Phones & Tablets — Through the Lifeline program, eligible clients can receive a free phone or tablet to stay connected.
Where We Serve
We currently operate in six California counties:
Los Angeles County (including Santa Clarita, San Fernando Valley, Antelope Valley)
Monterey County
Santa Cruz County
Contra Costa County
Riverside County
Merced County
Our teams are mobile — we come to you at shelters, hospitals, motels, encampments, and community sites. You don't need an office appointment to get help.

Experiencing homelessness or at risk of losing housing
Transitioning out of hospitals, jails, or institutions
Living with chronic health conditions and need care coordination
Uninsured or need help accessing Medi-Cal

What Happens When You Reach Out
When you contact us, here's what to expect:
A brief intake conversation — we'll ask about your situation, location, housing status, and what help you need.
We'll check your eligibility for CalAIM, Medi-Cal, and other programs.
You'll be assigned a case manager who creates a personalized plan with you.
Your case manager stays with you, following up regularly until you're stable and connected to everything you need.
