Job Description:
Mysti’s Adult & Family Services is seeking compassionate and qualified Case Managers to support individuals with developmental disabilities. This is an immediate hire opportunity, and candidates must meet staffing requirements.
Responsibilities:

Provide case management and advocacy for clients with developmental disabilities

Conduct intake assessments and develop individualized service plans
-
Connect clients and families to Regional Center and community resources
-
Maintain accurate and timely documentation standards
-
Coordinate with providers, clinicians, and other stakeholders to ensure quality care
Requirements :
-
Bachelor’s degree in Social Work, Psychology, Sociology, Child Development, Human Services, or related field OR equivalent experience
-
Minimum 1 year experience working with individuals with developmental disabilities
-
Knowledge of Regional Center services preferred
-
Strong communication, organizational, and problem-solving skills
-
Must pass background check and meet all compliance requirements

Reliable transportation (may include travel to client homes or community

Job Summary

We are seeking a compassionate and dedicated Nursing Assistant to join our healthcare team. The ideal candidate will provide essential support to nursing staff and assist in delivering high-quality patient care in various settings, including clinics, medical offices, and long-term care facilities. This role is crucial in ensuring the comfort and well-being of patients while maintaining accurate medical records and vital signs.
Duties

Assist patients with daily living activities, including bathing, dressing, and grooming.

Monitor and record vital signs such as blood pressure, temperature, and pulse.

Help with the mobility of patients, including transferring them to different locations within the facility.

Maintain cleanliness and organization of patient rooms and common areas.

Assist in the management of medical records and documentation to ensure accuracy and compliance with healthcare regulations.

Provide support in behavioral health settings by engaging with patients and facilitating therapeutic activities.

Collaborate with nursing staff to implement care plans tailored to individual patient needs.

Participate in training programs to enhance skills related to home community care, PICU, or other specialized areas as needed.
Qualifications
-
Previous experience as a Nursing Assistant or in a related healthcare role is preferred but not required.

Knowledge of vital signs monitoring and basic medical procedures is essential.

Familiarity with medical records management systems is a plus.

Strong communication skills and the ability to work effectively within a team environment.

Compassionate demeanor with a genuine desire to help others in their care journey.

Ability to adapt to various healthcare settings including teaching hospitals, clinics, or long-term care facilities.

Certification as a Nursing Assistant is preferred; however, candidates currently pursuing certification may be considered.
