Job Description:
Mysti’s Adult & Family Services is seeking compassionate and qualified Case Managers to support individuals with developmental disabilities. This is an immediate hire opportunity, and candidates must meet NLACRC staffing requirements.
Responsibilities:
Provide case management and advocacy for clients with developmental disabilities
Conduct intake assessments and develop individualized service plans
Connect clients and families to Regional Center and community resources
Maintain accurate and timely documentation per NLACRC standards
Coordinate with providers, clinicians, and other stakeholders to ensure quality care
Requirements :
Bachelor’s degree in Social Work, Psychology, Sociology, Child Development, Human Services, or related field OR equivalent experience
Minimum 1 year experience working with individuals with developmental disabilities
Knowledge of Regional Center services preferred
Strong communication, organizational, and problem-solving skills
Must pass background check and meet all compliance requirements
Reliable transportation (may include travel to client homes or community

Resident Attendant – Recuperative Care (CNA preferred) – Seaside, CA program site
Full-time, part-time and per diem | 8- or 12-hour shifts including awake overnights and weekends | $20 – $22 per hour; $22 – $24 per hour with an active California CNA certificate

Mysti's Adult and Family Services is opening a recuperative care program at our Seaside, CA program site. Recuperative care gives adults a safe place to rest and recover after a hospital stay. We need Resident Attendants on site around the clock. We interview this week and hire on the spot when it's a fit.
Pay

Schedule

Full-time, part-time and per diem

8- or 12-hour shifts: days, evenings, awake overnights and weekends

Overnights and weekends are the biggest need
What you'll do

Be present on site and awake throughout your shift

Complete and log wellness checks
-
Set out meals and snacks; check rooms and common areas
-
Sign in visitors; keep quiet hours and house rules
-
Call the site supervisor, on-call nurse or 911 per protocol
-
Help with rides to appointments; light housekeeping and restocking supplies
-
No hands-on personal care and no skilled nursing
What you need
-
18 or older, high school diploma or GED
-
Live Scan background check and TB clearance (we guide you through both)
-
Reliable transportation
-
BLS/first aid within 30 days of hire and de-escalation training (we provide both)
Nice to have
-
Active California CNA certificate (higher pay tier)
-
Experience in shelters, recovery, behavioral health or senior care

Bilingual English/Spanish

Mysti's Adult and Family Services is opening a recuperative care program at our Seaside, CA program site, where adults rest and recover after a hospital stay. We're hiring the person in charge on site: a licensed LVN, LCSW or MSW who is regularly on site during the week and reachable by phone outside those hours.
Pay (hourly, W-2)

LVN: $38–$42 per hour

MSW: $36–$42 per hour

LCSW: $44–$48 per hour

On-call stipend for phone availability outside scheduled hours; any call-in time is paid at your hourly rate
Schedule

About 20–25 scheduled hours a week on site, Monday–Friday daytime (for example 4–5 hours a day, or three longer days)

On-call by phone outside scheduled hours

Hours grow toward full time as the program grows
What you'll do
-
Lead the site: supervise resident attendants and oversee daily wellness checks
-
Coordinate intakes with referring hospitals and nursing facilities
-
Maintain care-plan and progress documentation
-
Coordinate with the visiting RN/NP and with case managers
-
Lead discharge planning into housing and community services
-
Infection control, incident reporting and program rules
-
LVN: observe medication self-administration. LCSW/MSW: lead the psychosocial side
-
No hands-on personal care
What you need

Active California license: LVN (BVNPT) or LCSW (BBS), or an MSW degree (BBS registration preferred)

BLS/CPR

Valid driver's license and reliable transportation

Experience with medically fragile, homeless or recovering adults
