If you’re struggling with housing—whether you need help finding a place, paying a deposit, affording rent, or staying housed—

We specialize in helping Medi-Cal members and community clients navigate complex systems and actually get results.

What Mysti’s Adult & Family Services Provides

Mysti’s offers a range of services depending on your health plan and location.


Housing Support Services (Community Supports)

Housing Transition Navigation

We help you:

  • Search for housing

  • Apply for units

  • Get approved

👉 Ask for this if you do not currently have housing or need help finding a place


Housing Deposits

We help pay:

  • Security deposits

  • Move-in costs

👉 Ask for this if you found housing but cannot afford to move in


Housing Tenancy Sustaining Services

We help you:

  • Stay housed

  • Prevent eviction

  • Work through landlord issues

👉 Ask for this if you are at risk of losing your housing


Transitional Rent (Critical Support)

👉 What it is:


Short-term rental assistance (often up to 6 months)

👉 What it helps with:

  • Paying rent while you stabilize

  • Preventing a return to homelessness

👉 Typical requirements:

  • You are homeless, at risk, or recently housed

  • You have a lease or are about to sign one

  • You need temporary financial support

👉 Ask for this if:

  • You moved in but can’t afford rent yet

  • You need time to stabilize income


Enhanced Care Management (ECM)

You receive:

  • A dedicated case manager

  • Help coordinating housing, medical care, and services

  • Ongoing support for complex needs

👉 Ask for this if you need long-term, consistent support


Community Health Worker (CHW) Services

Mysti’s provides direct support to:

  • Help you understand the system

  • Connect you to housing and services

  • Assist with applications and follow-up


How the System Works (Simple)

If you have Medi-Cal, you are assigned to a Managed Care Plan (MCP) such as:

  • Contra Costa Health Plan (CCHP)

  • Central California Alliance for Health (CCAH)

  • Other plans depending on your county

👉 These plans must approve services.

Important:


You must request a REFERRAL to receive services and be assigned to a provider like Mysti’s.


The Most Important Step: What to Say

When you call your health plan, be clear and specific.

Say this:

👉 “I am requesting a referral for Enhanced Care Management and Community Supports.”


👉 “I want to be referred to Mysti’s Adult & Family Services.”

If you do not say “referral” and do not ask for Mysti’s by name, you may be assigned somewhere else.


What to Ask For (Based on Your Situation)


If you need housing:

👉 “I am requesting a referral for Housing Transition Navigation”


If you need help moving in:

👉 “I am requesting a referral for Housing Deposits”


If you need help paying rent:

👉 “I am requesting a referral for Transitional Rent”


If you are at risk of eviction:

👉 “I am requesting a referral for Housing Tenancy Sustaining Services”


If you need ongoing support:

👉 “I am requesting a referral for Enhanced Care Management”


How to Request Mysti’s Based on Your Health Plan


Central California Alliance for Health (CCAH)

Mysti’s provides:

  • ECM

  • Housing Transition

  • Housing Deposits

  • Housing Tenancy Sustaining

Contra Costa Health Plan (CCHP)

Mysti’s provides:

  • ECM

  • Community Health Worker services

🕒 Monday–Friday, 8:00 AM – 5:00 PM

What to say:

👉 “I am requesting a referral for Enhanced Care Management and Community Supports”


👉 “I want Mysti’s Adult & Family Services”

Important:

  • CCHP may assign other providers for:

    • Housing Transition

    • Deposits

    • Transitional Rent

    • ECM is provided by Mysti’s only

You should still request those services.


Los Angeles County & All Other Counties

Mysti’s can still help—but you must contact us directly.

Important:

  • Do NOT call your health plan asking for Mysti’s

  • Your plan may not recognize Mysti’s as a provider in your county

  • You will not be referred to us through your plan
